The extra "Online Accounts" is from unity-control-center-signon
(specifically its credentials-preferences.desktop).
Do you have Unity installed?

gsettings get org.gnome.desktop.wm.preferences action-double-click-
titlebar
The output should be 'toggle-maximize'. If it is not, please run
gsettings reset org.gnome.desktop.wm.preferences action-double-click-
titlebar

returns which is the Online Accounts Preference Windows(Purple Vs.
Blue).
