The rhythmbox and system master volumes seemed to be linked before;
adjusting one would often adjust the other. Using rhythmbox
0.12.2-0ubuntu1 it appears this behaviour no longer occurs, so I believe
it's unlikely this crash will occur again. If it does I will reopen this
report.

My mistake, the volume weirdness still happens when PulseAudio is being
used. I had switched to ALSA.
