I've not been able to reproduce this so far on Karmic 9.10 x86.

hard copy without the use of OCR software, and the resulting image
converted to PDF.

Evince uses poppler to render text in PDF files. Changed package from
Evince to poppler.
I solved the rendering problem (missing text) by using Synaptic Package
Manager to install poppler-data. The file now renders fine, as far as I
can tell.
Not marking as invalid until we determine the best

data. Note that the error messages are gone, which correlates with the
correct rendering of the document.

With Adobe Acrobat Reader 5 I also see the box containing the message
Enable 3D View. According to http://en.wikipedia.org/wiki/PDF ,
Acrobat Reader 5 only supports version 1.4 of the PDF standard. I
believe that poppler (use by Evince to render PDF's) only suppors
version 1.5 of the PDF

I confirmed that the issue is in gstreamer as outlined above (actually
in Karmic one has to use the command $ gst-launch-0.10 ).
I then changed Firefox's Preferences Applications Action for the MIDI
audio Content Type to Use VLC Multimedia Plugin (compatible Totem
2.28.2) (in Firefox) to restore

I am no longer getting this with the desktop liveCD of Karmic 9.10
Release Candidate.
Uname: Linux 2.6.31-14-generic i686
The install no longer fails with an error near the end of the install
process, followed by returning me to the liveCD desktop. The install
now completes successfully and

an update to the latest kernel from the Karmic repositories (available
via Update Manager if not already installed) and an update of the
modem's firmware (try your mobile broadband company's website for this).
