been a while since bug was reported, there have been alot of changes in
Unity over the last couple of months. Can you please try the latest ISO
of the Ubuntu 12.04 beta and let us know if the issue have been resolved
for you.

This is a Unity bug. I've seen it happening for a number of my windows.
In fact, right now, it's happening to my Firefox window.

I just tried Geany a few times more and after 2-3 times the menu and
controls appeared in (maximized) top bar. It's frustrating to find bugs
like this, as you could see in the screenshot it is real, but I cannot
reproduce it.
