I ran several iterations of multiple instance testing across three
regions over the last couple of days (see [0]); all instances started up
first time which would indicate that this issue is resolved.

Thanks to bkrzem that posted a solution on a duplicate of this bug

Well, actually I've been struggling with this issue for a week! But in
my case the problem seems to be that nmbd is called to be started up
BEFORE the net device is actually on... so the solution in my case was
to change
ah... and this bug IS NOT a duplicate of bug #582376 (IMHO of course...)
as testparm was installed and perfectly working.

There are a couple of other bugs that cause this to happen if you have some
other services
installed. Some of these are fixed in uploads waiting on this SRU, and some in
yet to be
SRU'd fixes (which is why natty seems to always work).
Specifically,
Clint, I did have portmap running stopped prior on this test:
$ sudo service portmap stop
$ sudo service portmap status
portmap stop/waiting
$ sudo -i
# service portmap status
portmap stop/waiting
# apt-get install --reinstall libc6 shutdown -r now
on reboot
$ dmesg | grep orphan
again EXT4-fs
Booted to lucid (the one from comment #110 that showed no errors.
Install portmap repeated the test on that machine:
$ dmesg | grep orphan
