Two reasons. First, its functionality is bundled in current hotfixes. That is, you do not actually need qchain.exe itself because the hotfixes already do the "right thing". Second, I do not trust Microsoft to get this right. (Given that it was still broken until December 2002, I think my skepticism is justified.) Personally, I do not mind if the machine reboots 50 times while installing. Especially in exchange for the peace of mind knowing that the installation is as rock-solid as possible.
