A simple spread sheet is all you need - 5 columns , date, time,
stystolic, diastolic and pulse rate. In my case a a diabetic i also
have a column for sugar level. I can send you a copy of the form I use
if you want.

I have a report run from a push button on a menu form. I can print it
from the screen, but in Access I was able to program a push button to
print id directly. Is this possible in BASE? I cannot find any
