The Insert -- Horizontal Rule feature was removed at the 4.0.0 release, including its gallery of graphics objects. And it was not replaced. There is an active bug fdo#60858 https://bugs.freedesktop.org/show_bug.cgi?id=60858 , and discussion in the UX-Advise
