Am I missing something? I note that in Open Office 4.0.1 the feature
of inserting footnotes directly from the Endnote Bibliographic
Database is missing. I could do this in Open Office 4.0.0.

Ungrade your OO and/or OS to minor version. It should to solve the problem.
