When you set up an appointment in the calendars app, there is an option to set
an alert. Once you set 1 alert, for up to 2 days in advance and press the done
button, an option to set a second alert is presented. Double tap it, and the
same options will be available, from 2 days

When you set a calendar event, their is a setting marked ALERT.
You can set the alert for the times specified by the choices.

How did you set up the appointment? Did you use the Calendar app?
I use Google Calendar as my default calendar, and when I set up an
event, I have two alerts I can set for each event. If you've done this,
and you're still not hearing an alert, you may want to go into Settings
