Might be similar, I finally set up a new si server box from scratch and everything worked fine, but the package versions were the same as what was on my old box (unless I am remembering wrong). I then re-installed my original si server and it also works fine.

Did you update recently? I hade this problem after a recent update on debian, and the network boot kernel no longer had ide-scsi. I found the old kernel on a backup (after trying to fix by recompiling, etc, but failing). You can try edit the install script to give some feedback (lsmod, modprobe ide-scsi, etc) just before the point of failure.
