If your requirements is different from http_access then rewrite.
Or you could change icap_access to work similar to delay_access. Instead
of using a different access list, use one access list per icap class, and
evaluate them in the order the icap classes are defined. The first allowed
to be used is used. Drawback is that this is quite confusing to users,
benefit is that it fits quite nicely within the existing code.
