Q1         How long can you livewith heart disease and diabetes?

A1          Dr.Ganeshakrishnan Iyer,MBBS, MS, MCh. Lead Consultant - CTVS
Surgery17h

There are two important factors to consider when you are asking how longyou can
live with heart disease and diabetes - yourage and state of health.

Most heart-disease patients live for more than 10years after the diagnosis, so
itis common for people to live a long life, even after being diagnosed with
heartdisease.

Although you may be diagnosed with diabetes, it does not mean that yourlife is
in danger. The most importantthing is to follow your doctor’s recommendations.
He or she will determine whetheryou have to follow any specific diet or will
have to take any medication. Every case is different, so it’s good to discuss
your specific casewith your doctor.

Q2         Why does the pH of bloodneed to be kept constant? What happens if it
is too low or too high?

A2          Ken Saladin,Textbookauthor and professor emeritus Mon

Most of our body’s functionsare controlled directly or indirectly by enzymes.
Enzymes are proteins thatfacilitate chemical reactions in the body. Their
ability to do that depends on their ability to bind and releaseother molecules
(their substrates and products), and that, in turn, depends ontheir precise
three-dimensional shape.

Roughly (though there are some weaknesses in the analogy), the relationship of
a substrateto its enzyme is like the relationship of a key to a lock. In the
figurebelow, the “lock” is blue and the “key” is red. If a lock is slightly
changed, the key won’t fit andcan’t do anything. The enzyme is then said to
bedenatured. If an enzyme’s shape is slightly changed, its substrate won’t
fitand the enzyme can’t do anything to it.

The shapes of enzymes and other proteins are very pH sensitive. If the pHshifts
too far away from the norm, the enzymes change shape, the substratesdon’t fit,
and metabolic pathways become dysfunctional or nonfunctional. This really
bollixes up the organism’s metabolismand cellular function, potentially to the
point ofconvulsions, coma, or death.

Heat (such as high fever)denatures enzymes too; thefollowing diagram could
equally well depict heat or pH effects. Proteindenaturation by heat is why egg
white turns white and opaque when we cook it.Putting egg white in acid will do
the same.

One tragic example of the pHeffect is death from eclampsia, a disease in which
some pregnant women vomit soabundantly and for so long that the loss of stomach
acid shifts their body pHinto a fatally alkaline zone.

Diabetes mellitus, if inadequately treated, shifts the body pH the otherway,
into the acidic zone (diabetic acidosis, or ketoacidosis—the latter namesince
the acid comes from waste products called ketones).

The normal blood pH is 7.35 to 7.45. A pH below 7.35 is called acidosis, and
below 6.8 is usually fatal. A pH above 7.45 is called alkalosis, and above8.0
is usually fatal.

To prevent this, the body haspH-stabilizing physiological and chemical buffer
systems. The fastest-acting chemical mechanism is chemicalbuffers:
bicarbonate-carbonic acid, phosphates, and proteins that bind orrelease
hydrogen ions. The fastest-acting physiologicalmechanism is the respiratory
system, increasing or decreasingbreathing rate and depth to adjust the body’s
carbon dioxide level (CO2generates acid in the body fluids). The respiratory
system can neutralize 2 to3 times as much acid as the chemical buffers do.
Theslowest-acting but most effective over the long term is the urinary
system, which continually regulates pH by excreting acid in the urine.

Hence the urine pH averages about 6.0 (mildly acidic), but it can beadjusted
over a range from 4.5 to 8.2 to compensate for such pH imbalances aspeople get
from diabetes, emphysema, pre-eclampsia, asthma, diarrhea, alcoholism, and
myriad other causes. This is areason urinalysis is a routine part of periodic
medical examinations; urine pH(among the many other things measured) can be an
important warning sign ofmetabolic dysfunction.

My note- An excellent explanation telling importance of pH value in bloodand
urine

Q3                 What will happenif I only eat fruits and vegetables every
day?

A3          Charu,Former Fitnesstrainer22h

Your system will be floodedwith micronutrients.

Vitamins ,minerals, antioxidants, fiber et cetera.

When you hear the term micronutrients, you may not take it seriously-because we
think that it is micro.

Your body may not drop amessage,Like hey charu, today you have prevented your
body from chronicinflammations, like all other day,you protected yourself from
getting cancer ( again this doesn't mean those whoare diagnosed with cancer
aren't eating healthy, there are tons of reasons whypeople suffer from cancer.)

Or it may not tell us, hey because of your consistent consumption ofveggies and
fruits on a regular basis. Your gut health, stamina, mood swings, joint
health, bone, eyes, sensory organs are working optimally.

I wish our bodies would drop a message, whenever you consume food whichare full
of micronutrients! So that, everyone start consuming loads of fruitsand
vegetables!

If you eat fruits and vegetables everyday, you are increasing yourlifespan, you
are preventing diseases, that makes you a wise human being!

Remember, PREVENTION IS BETTER THAN CURE.

Today's breakfast along with eggs! One remarkable fact about eggs:Itcontains
VITAMIN-D! ( TADA)

Along with fruits and veggies, you should include macro nutrients too. (
protein, carbs and fat)

Q4         Would you suggest takingan AC coach in Indian trains in the winter?

A4          Shubham Phatak, Lives inNavi Mumbai, Maharashtra, India
(2016–present)12h

If you are not in AC coach in winters Be ready for chilly winds gushingout
through doors and windows gaps. Contrary, you will be in maintained 16–20°
approx. AC compartment and nowinds at all. You can even close air vents by
pulling small levers nearroof. Side upper seat has this personalised perk as
for other side, you willneed to ask everyone before closing.

When you are in deep sleep, security becomes an important issue to becareful
about, especially in single track lines where crossing related haltsare
frequent.

You won't be getting bedsheet, blanket and pillow unlike AC coach.

But you will save a lot ofmoney by opting non-AC compartment.

Also you mightn't need AC if you are from deep south or coastal areaswhere
temperature range is narrow.

So, if you are from Himalayanstates of India, northern Indian plains, North
East areas or plateau regions like Maharashtra, Telangana, you can optfor AC to
avoid aforementioned disadvantages.

Q5         Tirupati, Andhra Pradesh,India: Why do so many devotees flock to
tirumala?

A5          Hitesh Mohan,Lives inTirupati, Andhra Pradesh, India
(2019–present)Nov 12

Because of the history anddivinity associated with it. Trust me you will never
find a single Teluguperson who never visited Tirupati in his/her life. The
popular assumption is that the Venkateshwara swamy idol inTirupati is swayambu
(self born) and it is the place where Lord Vishnu placed his foot in KaliYuga.
This is the reason why lakhs of devotees visit Tirupati everyday.

There are many legends behind the history of Tirupati.

Once Bhu Devi requested Lord Vishnu that she is not able to bear theweight of
sinners on the Earth in Kali Yuga and to protect the Earth from thesinners. To
which, Lord Vishnu assured Bhu Devi (Goddess Earth) that he will stay on the
Earthfor entire Kali Yuga to protect her. He choose the 7 hills of Tirupati as
hisabode , this is the reason why Tirupati is called Bhu Loka Vaikunta.
Acoording to Varaha Purana, Venkateshwara swamy will reincarnate asKalki at the
end of Kali Yuga from the Tirupati Balaji idol.

The another legend says, the foster mother of Sri Krishna Yashoda Deviwas sad
for not witnessing anyone of Krishna’s marriages and complains the sameto Lord
Krishna. To which, Lord Krishna promised her that she will get theopportunity
to witness his marriage in Kali Yuga. Hence inKali Yuga, Lord Krishna
incarnated as Venkateshwara in Tirupati, GoddessLakshmi as Padmavati in
Tiruchanur near to Tirupati and Yashoda Devi as VakulaDevi who later became
foster mother to Lord Venkateshwara. As perthe divine plan, Venkateshwara and
Padmavati fell in Love and married each otherin front of Vakula Devi. Itwas
said that Lord Venkateshwara had taken a loan of 15 million Rama mudrikagold
coins from Kubera for his grand marriage and promised him that he wouldrepay
the loan with interest by the end of Kali Yuga. This is becausePadmavati devi
was born in rich & royal family whereas Venkateshwara swamychooses to be poor.
Their marriage happened on the 7 hills of Tirupati and allGods (Shiva,
Brahma,Ganesha, Karthikeya, etc….) attended the Lord’s wedding.Vakula devi
witnessed what she wished. After the marriage, Lord Venkateshwaraand Padmavati
Devi spent several years on the 7 hills of Tirupati. This is whyTirupati got
another name Bhu Loka Vaikunta.

This is the reason why many devotees flock to Tirupati . It is theworld’s most
visited pilgrimage site and no wonder it is the richest temple inthe world.

So, when you visit Tirupati Youmake sure that you first visit Padmavati devi
temple in Tiruchanur ( 10 KM from Tirupati) first and then visit Vakula
MataTemple and finally visit the Lord Venkateshwara swamy Temple.Om

Q6         What is something that isabsolutely worth the money?

A6          T.M. Gautham,Left Handed& Right Minded5y

Bicycle.    It has no expiry date.    Traffic police won’t stop you.   Good for
both physical and mental health.    Helps to keep your tummy in size.    No
carbon footprint.    No noise pollution.    Occupies less space in parking
lots.    Less expense on maintenance compared tomotorbikes and cars.    Can
sneak intosmall gaps in heavy traffic.    Bicyclingis cool.    Bicycling is fun.

My note-After studyingbicycling by my grand children, the regular cycle is kept
in my portico. When my son comes he used to ride onit to near by shops. The
clothes taken from clothes line in portico is firstput over cycle and then
together taken inside.

Q7         Why do some people travelin 1A (first class) train instead of taking
a flight, which has almost same orslightly more fare?

A7          Vijay Srini,Lives inBengaluru, Karnataka, India19h

These are the reasons.

Luggage: By flight, you areonly allowed 15 + 7 kg, orelse you have to pay more.
In 1st AC, you can carry upto 60 kg.

Certain items are notpermitted by flight. They are more easier to carry by
train, barring of course, explosive or dangerous items.

Airport requires too muchsecurity checkin, due to which you need to arrive 2
hours before flight, maybe 1 and half hours for short distance, but notsure
(here in India, in other countries, even one hour is sufficient).Nevertheless,
you need to arrive well in advance. And in cities like Bengaluru where the
airport is 35 kmaway from the city center, travelling could be a nightmare. So,
for mostpeople, air travel will not be feasable for travel less than 500 km,
unless theperson is staying nearby the airport. To travel by train, even
arriving 30 minutes before issufficient.

Q8         What are the financialbenefits of Indian Railways selling tickets
for its lowest class of travel, ACsleeper and second-class seats?

A8   Mahabaleshwar Kulkarni,FormerRetired Deputy Tahsildar. at Karnataka
Revenue Dept (1971–2008)22h

There is no financial benefit in selling general class seats and secondclass
seating. These seats are provided for the benefit of poor people, atsubsudised
rates. Torecoup loss and generate money AC classes are being provided.

Q10               Do trains need tobe braked at every station even if it is
standing still?

A10        Gary McAdam, Amateur railhistorian and frequent rail traveller12h

Yes - though it’s possible fora train to stop at a platform without the brakes
being on, the doors won’t release on modern stock if the brakes aren’tapplied
(they’re interlocked with both the brakes and traction power). Thereare ways &
means of isolating individual doors if the interlock’s lost, thetrain has to be
taken out of service as soon as practical if that happens.
