I was receiving the same error with trac 0.9.5 on apache 2.0.54, python 2.4.1, mod_python 3.2.8, sqlite 3.3.6, and pysqlite 2.3.2.  The culprit appears to be pysqlite.  When I downgraded the pysqlite to 2.1.3 (the version reported on trac's site to be the most stable) everything straightened itself up.
