I am experiencing a very similar, if not the same, problem. I'm running
the latest Jaunty kernel (2.6.28-11-generic). Occasionally, after
resuming from suspend-to-ram, my internal iw3945 nic will not respond -
the wlan0 device does not exist. The problem persists after removing and
re-inserting the iwl3945 module. Here are the relevent messages from
dmesg:

slightly different from  what's posted above, but the symptoms are the
same. The only way I can figure out to re-enable the wireless is to
reboot.
