It appears this was a odd relic issue after upgrading the server from
Gutsy (now unsupported).
Issue was resolved by:
$ sudo apt-get purge postfix
then
$ sudo apt-get install postfix-pgsql
** Changed in: postfix (Ubuntu)
Status: New => Invalid
