Yes, I have configured Phonon correctly, although Amarok 1.4 is from KDE3 and doesn' t itself use Phonon. My usual quick fix when the sound ceases is to change the output device in Amarok to Alsa and then back again to PulseAudio.
One thing I forgot about when I filed this report was that the sound in some games also dies, usually very quickly. I tried running Amarok and Super Tux Kart at the same time. The first time the sound died in STK while the opening menu was still displayed and no processor-heavy graphics stuff was going on, but the sound from Amarok carried on.
