Works fine for me with the following pxelinux configuration and the
attached preseed file:
DEFAULT preseed
LABEL preseed
kernel linux
append vga=normal initrd=initrd.gz DEBCONF_DEBUG=5
debconf/priority=critical url=http://10.153.104.60/cgi-bin/preseed.cgi
locale=en_US console-setup/ask_detect=false console-setup/layoutcode=us
hw-detect/start_pcmcia=false netcfg/choose_interface=eth0
