I've determined that it was a hardware failure.
The motherboard in question (ASUS M2NPV-VM) used an nVidia chipset that
had a known manufacturing defect (the BGA contacts on the southbridge
lift after a number of heat/cool cycles, causing intermittent/incomplete
continuity). The problem was never properly addressed by the motherboard
manufacturer (Asus), so I replaced the motherboard with an MSI
785GTM-E45 and the problem was immediately resolved (with no change to
any software, including OS). This bug can be cancelled/deleted/closed.
