It looks like this is a duplicate of bug 432964:
groupadd: cannot lock /etc/group; try again later.
adduser: `/usr/sbin/groupadd -g 123 couchdb' returned error code 10.
Exiting.
It seems that some earlier crash of some shadow util has left
/etc/group.lock file. In case nothing updates this file then the lock
file needs to be removed manually.
