I never use the graphical interfaces for updating; I use the command- line 'sudo apt-get update' and 'sudo apt-get upgrade'. This never updates the indicator-session status, so for me it always reads "Updates Available...". However, if I click that, it pulls up the update manager and tells me there are no updates, and then finally updates the indicator.
Definite agree with #2.
