I think the problem may lie with the symlink. ls -la /usr/share/pixmaps/f* lrwxrwxrwx 1 root root 44 2012-01-11 22:54 /usr/share/pixmaps/firefox.png -> ../../lib/firefox-9.0.1/icons/mozicon128.png
Does not seem to correctly map to /usr/lib/firefox-9.0.1/icons so the link probably just needs to be rebuilt. The question is why do I need to do it?
