I suspect restarting pulseaudio would resolve the problem as well,
making it hard to catch. The best thing would be if you could catch a
disappearance while a PulseAudio log is running (see
wiki.ubuntu.com/PulseAudio/Log ), but that might be tricky. Could also
be interesting if there is anything of interest in syslog when a
disappearance happens.
