What I find odd it that your getting a duplicate error in the sources file. That should definitely not be the case with a fresh install. Had you release upgraded that machine in the past?
Also attach the most recent logs. (/var/log/dist-upgrade) Looking at your older ones, I didnt see an error in regards to upgrading.
