I have had this bug since I installed 16.04 (Not an upgrade) It does not happen often but when it happens it is always when returning to my normal user id after having switched to another user id.
So basically I click upper right corner and selects another user, say user2, when I am logged in as user1. When I am finished with user2 I just log out and am normally returned to the login window where I can login as user1 again and the session is resumed.
