The debconf question is not asked when installing with jockey (-gtk or -kde), so nothing gets downloaded and there are no files in /lib/firmware/b43*.fw.
config.dat has Name: b43-fwcutter/cut_firmware Template: b43-fwcutter/cut_firmware Value: Owners: b43-fwcutter running dpkg-reconfigure b43-fwcutter lets you chose to download it and then it works fine.
