Hello Kevin, If I had to to something like this I would also probably define a frame anchored to the header and aligned to the outer margin of the page with, inside the frame a table with 1 column and as many rows as I have chapters. If the chapters are numbered you may be able to use some clever trick playing with conditional text and conditional styles to make some big black spaces with black highlighting appear only in the row corresponding to the chapter number. I have tried for 10 minutes or so but did not manage to do much, on the other hand I have almost never used the conditional text so there may very well be a way and I do not know it.
