I see you have worker.xxx.host=server_name
Make sure the mod_jk can resolve the server_name
to a correct address, and that your firewall (if set)
allows those ports. For performance reasons it
might be better to use IP addresses instead server names.
Also set JkLogLevel debug and see what's going on.
