I was seeing this exact same problem last night. It turned out
that there was a problem with tcp.smtp.cdb being updated. I saw
nothing in the logs either. There were a load of
tcp.smtp.tmp.XXX lying around in /var/spool/pop3/etc. I did some
cleaning up and that seemed to fix things.
