A simple test database in 4D v15.4. The data comes about because the province 
of Nova Scotia has decided to change their billing codes and diagnostic codes. 
As a result, I have base data to play with.

16 tables. 5 for SNOMED diagnostic codes, 11 for CPT fee codes.

Only looking at two of them. One is called ‘members’. The other is 
‘Members_Hierarchy’.

I setup a ‘query’ form to have a look as to how the two tables interrelate. For 
the SNOMED codes, all is good. I can trace up the hierarchy easily.

For the CPT codes, I created a query form, and using the system created forms, 
I drug both the members output form and the member hierarchy output form onto 
the query form so the results of a query will be listed in the subform.

The members list shows and will populate when a query is done. The hierarchy 
form will not display - not even the headers or the surround box.

The results of the query do show in user mode when that table and the output 
form is selected, but not as a subform on the query form.

Any ideas?
