Two thoughts:
1) You have a third party application installed such as Intellimouse
or Mouseware.
2) Your USB support drivers are corrupted.  Try installiong the
Windows SE Service Pack (unnoficial).  This has a wide array of USB
drivers included in it and might rewrite the bad file(s).
