I personally would define the rule to be:  All octaves are treated 
like the first specification, unless explicitly defined otherwise.

Thus, K:^f=F would mean f's in all octaves are sharp except for F.
And K:^f^c=F would mean f's and c's in all octaves are sharp 
except for F.
