Fix lots more bugs in header/footer code. Can now have headers and footers
in a document. Change all page margins/page sizes on the fly with
headers/footers. Still need to fix undo and put in code to deal with
people inserting a footer into a doc that already has a footer.
Also put in code to reserve the first 10000 id's for our own purposes. ie
Numbered Headings..
Finally added a little bit doxygen comments.
