I haven't tried out your patch, but from your description, it sounds like
the functionality is a little 'confused'.

In most word processors, 'Ignore' means "Ignore the currently highlighted
instance of this misspelled word".  'Ignore All' means "Ignore all instances
of the currently highlighted misspelled word".  There is no option to ignore
all misspelled words (that's equivalent to switching off spell checking).

Apologies if I've simply read your mail wrongly and that's what your patch
actually does.
