This sounds similar to a problem we experienced recently due to a bug in a Juniper router's PIM implementation. The router would delay the S, G JoinPrune messages so the upstream neighbor would tear down the shortest path tree, disconnecting the session. When the JoinPrune messages did finally appear, things got rebuilt and the session was joined again with working audio & video.
Do you have a multicast beacon set up (http://dast.nlanr.net/Projects/Beacon/<BLOCKED::http://dast.nlanr.net/Projects/Beacon/>)? Someone who knows multicast should be able to monitor the beacon data and troubleshoot your multicast problem.
