Issue of Photo Identity Cards for Physically Challenged persons over Vijayawada Division Physically Challenged persons under the jurisdiction of Vijayawada Railway Division Viz. Nellore, Prakasam, Guntur, Krishna, West Godavari, East Godavari and Visakha Districts. Physically Challenged persons under the jurisdiction of Vijayawada Railway Division entitled for Railway Concession should approach the Chief Booking Supervisor of nearest nodal Railway Station viz. Nellore, Ongole, Tenali, Eluru, Rajahmundry, Samalkot, Tuni, Bhimavaram Junction. Gudivada and Commercial branch, Divisional Railway Manager’s Office, Vijayawada along with a requisition letter with the following documents. 1. Railway Concession Certificate obtained from the competent Doctor/Hospital within the jurisdiction of Vijayawada Division ( Blind persons can also obtain recognized by Ministry of Welfare/Govt. of India or social Welfare Dept. of the State Government) Certificate should be issued as per the norms in force as decided by Railway Board. i.e. seal of the doctor should contain name of the doctor, designation, registration No. along with the hospital address. ( The Certificate in which name of the doctor, designation, registration No. in ink/hand writing should not be accepted.) 2. Disablility Certificate issued by Govt. of A.P. or Medical Board. 3. Valid Photo Identity Proof viz. Voter Identity Card, Passport, PAN Card, Driving Licence, Aadhaar Card. 4. Date of Birth Proof. 5. Valid Address Proof and 6. Two recent passport size photographs (Chest level only) 7. One recent Stamp size Photograph (Chest level only) Either in person or by post for issuing Railway Photo Identity Card by submitting three sets of self attested photocopies of the said documents. The covering letter should be addressed to Chief Booking Supervisor of nearest nodal railway station viz. Nellore, Ongole, Tenali, Eluru, Rajahmundry, Samalkot, Tuni, Bhimavaram Junction, Gudivada, & Commercial Branch, Divisional Railway Manager’s Office, Vijayawada. If the application is sent by post, the envelop containing the documents and covering letter should be super-scribed as “ application for issue of Railway Identity Card for Physically Challenged persons for ticketing”. The Physically challenged person should provide their mobile number/landline number for communication while applying for the card. Applications in person will be accepted on all working days ( Monday to Friday) between 10.00 hrs. to 14.00 hrs. only at locations other than Divisional Office, Vijayawada. Only non-laminated, concession certificate obtained from competent authority lying in the area within the jurisdiction of Vijayawada Railway Division will be considered for issuing photo identity cards.
