I come across about it in news sometime in January-February. If I
recollect rightly, the major change is that the indian railway has
changed the nomenclature from viklang to Divyang in the concession
form.
But where and when it will be available in public domain is obscure.

Amiyo da, are you referring to the format issued in July 2015 by any
chance? To the best of my information and knowledge, this was the last
revised format.
