Yes I have experience the same.

When I book the tickets from Mumbai to Ahmedabad in train number 12901 in 
handicap Kota in third AC, I got one ticket confirm and another waiting.  note, 
both passengers are disabled.
when I book the ticket online it was showing to tickets available but when I 
made the payment one ticket was waiting another one is confirmed Sent from my 
iPhone
