This is complicating. They want the UDID no. and also the name of the doctor signing the medical certificate and the name of the hospital. I thought all this data would be available to them once they feed in the UDID number. This is ridiculous and unnecessary. Not sure if most of us have access to the name of the Doctor who might have signed the medical certificate. This is bureaucracy for you. Good luck to all!
