In my case, the certificate or UD ID card is issued from Minto Hospital Bangalore. When I open Dr diagnostic sheet. It is written only the designation of the doctor that is chief medical officer, but there is no name of the doctor so we again we are finding the same problem. Some places, doctor name is at most places. Doctor name is not available.

Yes now it is mandatory UDID number and UDID medical certificate is must if you want to avail 80U & 80DD in filing ITR.
Is it mandatory to have a unique disability identity card in order to file form 10EA? As I do not possess unique disability card as of now Sent from my iPhone This is complicating. They want the UDID no. and also the name of the doctor signing the medical certificate and the name of the hospital. I thought all this data would be available to them once they feed in the UDID number. This is ridiculous and unnecessary. Friends as I have mentioned in the subject for availing 80U or 80DD while filing it is now mandatory to fill 10IA form in Income Tax website, yesterday I myself tried to fill 10IA form but the form was not properly accessible there we have to fill UDID Number issued by UDID authority, Name of Doctor signed in UDID Medical Certificate, Name of Hospital, and also have to upload UDID Medical Certificate, and at last we have to othenticate with Aadhar card through OTP , and after submitting we will get acknowledgement number and date on our mobile through SMS. Today I filled 10IA form with the help of computer operator in my office, now my question is so much detail is necessary? just providing UDID number is not enough otherwise we can even upload our UDID medical certificate, why this 10IA for required, income tax officials before introducing this form whether they had consulted any disabled NGOs or any accessible testors of screen readers,, my request to legal experts to take this matter in front of income tax officials. let me see how many members will comment on 10IA form
