I selected multiple disability as somebody on the list suggested. As regards the doctor's name, the certificate I downloaded from https://www.swavlambancard.gov.in/home/login does not contain any doctor's name. But I downloaded another version of the same certificate from the state government's site which contains the doctor's name and also the CMO's name. I uploaded this certificate.
I found my UDID number in the place for certificate number in both certificates. So inserted this number.
