hello everyone, I have also applied for my UDID card. It asked me to enter the dr's name, for the CON SERT. So, I entered it from my diagnostics sheet. But, the registration number was not given on that sheet or my disability certificate and not on my UDID. So, I mentioned NOT on CERT. as for the date, I filled in the moast acuraite date I knew. And,yes, I have attached the aadhar updated UDID there. I hope it helps.
