Not on UDID card but disability certificate must have it Sandesh.
Check the original notification by which the format was notified. It is
a laps on part of the hospital concerned.
Up to everyone to decide ultimately.

Persons with various disabilities are happily benifitting by travelling
in almost all states using this UDID card, many people are employed as
disabled, Pan India with UDID card and disability certificate as
concrete proof of their disability. Why is such intimidation then?
The Department of Empowerment of Persons with Disabilities (DEPwD),
Ministry of Social Justice & Empowerment is distributing this card for
at least last 10 years now. Why even they did not feel any need of
keeping these things printed on UDID card? I'm not sure but it appears
that Even disability certificates do not have this doctor name and
registration number on it. Are we going to suggest to the department of
Ministry of Social Justice & Empowerment to issue fresh UDID cards and
disability certificates with doctor name and registration number
printed on it just for the sake of simple Railway application error?
In short, instead of making things complicated, we should try simple,
suitable and feasible approach.

them if one (blind person) applies using UDID, these columns of

certificate is taking us towards UDID, what is printed on it, doctor
diagnostic sheet, about Aadhar kyc UDID etc etc. The focus of the
discussion goes wayward, which shouldn't be the case.

As Sandesh ji Mentioned, finally, he managed to upload his UDID card
and was able to submit the application for E-concession for railway.
But, in my case, Doctor's name is not there on my UDID card or neither
in Doctor's diagnosis sheet. I don't know why is this. But, without
adding the registration number and the other offline concession
details, the portal is not allowing me to submit the data.
I kindly request Sandesh ji or anyone who applied for the railway
Concession through UDID to guide me for doing the same.
