Dear Turab,
You need to visit the hospital selected by you when filling the online
UDID form for a medical evaluation. The medical evaluation report is
uploaded by the hospital on the UDID website & only then the UDID
certificate is issued.
I had to visit my selected hospital 3 times - once to get an appointment
for medical evaluation, once for evaluation by a junior doctor & once
for evaluation by a senior doctor.
