my case, since my disability certificate was obtained off-line and that in my 
hometown as in, I am currently in a different location, the hospital was chosen 
by me. During the application stage. I will take my application form with me 
and my disability certificate issued earlier and explore what has to be done.

I wanted to share my personal experience, which might help clarify the 
procedure. I submitted my UDID application on September 15, 2025. Initially, I 
was confused whether the hospital would call me for verification or if I should 
visit them directly. After waiting for a month without any communication, I 
decided to visit the hospital mentioned in my application. Here is what I 
learned from my experience: Direct Visit is Better: Don't wait for a call from 
the hospital. If your status shows 'Verification Pending,' it is better to 
visit the hospital directly. When you visit, ensure you carry the Original 
SADAREM Certificate and the UDID Application Receipt. In my case, the officials 
cross-verified these documents, and the entire process took only about 5 to 10 
minutes. A key point to remember is that we cannot choose a hospital of our 
preference during the UDID application if we are applying via a SADAREM 
certificate. The application is automatically routed for verification to the 
same hospital that issued your SADAREM certificate. Once the verification was 
completed on October 17, 2025, I immediately received a confirmation SMS. 
Shortly after, when the card was generated, I received another message with my 
UDID card number. While the soft copy can be downloaded instantly from the 
portal, the physical (hard) copy arrived via post by November 10, 2025 (it 
usually takes about 2-3 weeks). I hope this information helps anyone who is 
currently stuck at the hospital verification stage. Thank you Best regards B.

form for a medical evaluation. The medical evaluation report is uploaded by the 
hospital on the UDID website & only then the UDID certificate is issued.


I had to visit my selected hospital 3 times - once to get an appointment for 
medical evaluation, once for evaluation by a junior doctor & once for 
evaluation by a senior doctor.
