Ministry of Social Justice and Empowerment Government of India Invitation of Applications for the National Awards for the Empowement of Persons with Disabilities - 2008
Applications are invited for the national awards for the empowerment of persons with disabilities under the following categories:- (i) Best Employees (including self-employed) with disabilities; (ii) Best Employers and Placement Officer/ Agency for placement of persons with disabilities (Govt. Sector, PSU and Private Sector); (iii) Best individual (including professional) and best institution working for the cause of persons with disabilities; (iv) Best Technological Innovation for the cause of disabilities and adaptability of innovation to provide cost-effective technology; (v) Outstanding Creative Adult Persons with Disabilities; (vi) Outstanding Work in the creation of Barrier-free environment for the persons with disabilities; (vii) Role model (viii) Best District in providing rehabilitation services; (ix) Best Local Level Committee of the National Trust for Welfare of Persons with Autism, Cerebral Palsy, Mental Retardation and Multiple Disabilities; (x) Best creative children with disabilities; and (xi) Best State Channelising Agency of the National Handicapped Finance and Development Corporation. 2. The applications of those employed in the Central/State Governments/Union Territories/Public Sector Undertakings should be sent in the prescribed proforma (either in Hindi or in English only) to the Section Officer (DD.V), Ministry of Social Justice and Empowerment, Room No. 622, 'A' Wing, 6th Floor, Shastri Bhavan, New Delhi - 110001 through the concerned Ministry/Department/State Government/ Union Territory Administration/Public Secor Undertaking duly approved by the competent authority. 3. In case of others (including self-employed persons, unemployed one and those employed in private Organisations/unorganized sector), applications should be routed through the Social Welfare Department or such Department of the concerned State Government/Union Territory Administration, which is concerned with the welfare of the persons with disability. Applications other than through proper channel shall not be entertained. 4. The proforma for the application can be collected from the Section Officer (DD.V), Ministry of Social Justice and Empowerment, Room No.622, 'A' Wing, 6th Floor, Shastri Bhavan, New Delhi - 110 001 (Telephone no. 011-2338 6314, FAX no.011-2338 4918) or the same can be downloaded from the Ministry's website http:\\ www.socialjustice.nic.in . Suitable photographs may also be attached with duly filled in application forms. 5. Following documents should be enclosed with the application for the Award:- (ii) Summary of bio-data. (iii) Material for Citation. 6. The last date for receipt of applications is 1st September 2008. Applications received after the expiry of the prescribed last date and/or not recommended by the prescibed competent authorities wil not be entertained without any further reference in the matter.
