To, The public information officer, Department name and adress here. Subject should be clear and not generic. Such as seeking the information under the RTI act regarding the xxxxxxxxxxxx Then start your application like any other application. Sir/Madam, Furnish the information sought as under. Start asking your questions Para wise divided in numbers. Avoid questions amenable for 'Yes' or 'No'. In the last Para declare that I am enclosing the Postal order of Rs. 10/- baring no. - ABC. This is very important. Now end your application like any other application and provide your address, email ID and phone number. should you require, I will try to find some RTI drafts which I sent to various authorities from time-to-time.
