I launch the shell and then my browser with http://localhost:8080.

The demo structure uses port 8080 to listen on but the regular shell uses port 80 (the default), so just use http://localhost.

The Active4D.ini file contains the file extensions .a4d, .a4p and .htm

I would not include .htm as executable in Active4D.ini. You want to keep that for static pages.

It took me quite a while to discover the a4d file extension and to understand why the default.htm page was not the one displayed.

Yes, I can see I did not clearly document that in the "HTTP Server" chapter. I will fix that.

Zach Dennis told me how to make a4d pages in Dreamweaver, but I haven't found out how to get them to display in design view, which I am told is possible.

On the Active4D download page at the bottom are links for downloading the Dreamweaver MX integration tools, which has all the files you need with detailed instructions.
