I'm awayfrom my documentation at the moment but I believe the
default 'home page' is 'default.a4d' not 'index.a4d' if I remember
correctly.
Also make sure you don't have index.a4d.a4d (This caught me out in
the past).
You can change the default home page in the Active4D.ini file.

I have index.a4d specified in the Active4d.ini file.
Still a 404 from A4d.
