I ran into the same issue. I tried everything I could think of, but could not solve it. Ultimately, I worked around the problem by putting a sequence number in the PDF file name. I'd love to hear of a better way.
Then that means it's a caching issue. IE is notorious for (among many, many, other things) for not listening to no-cache directives.
